David Bates, MD, MSc, Director of one of three national Centers of Excellence in Patient Safety and Research supported by AHRQ focusing on improving medication safety across the continuum of care and patient groups. Since 1998, he has served as chief of the division of general internal medicine at Brigham and Women's Hospital. A practicing general internist, Dr. Bates is also medical director of clinical and quality analysis information systems for Partners HealthCare. He is a professor of medicine at Harvard Medical School and professor of health policy and management at the Harvard School of Public Health, where he is co-director of the program in clinical effectiveness. He is also chair of the National Alliance for Primary Care Informatics, a coalition of primary care societies working to increase the use of electronic health records. He works closely with the Leapfrog Group on computerized physician order entry (CPOE). Trained as a clinical epidemiologist, Dr. Bates' main research interest is the use of computer systems to improve patient care. He evaluates the incidence and preventability of adverse drug events (ADEs) and also works on improving the systems by which drugs are given. Dr. Bates also leads a series of studies on improving efficiency and quality using information systems more broadly. His awards include the first John M. Eisenberg Award for Excellence in Patient Safety Research from The National Joint Commission and Quality Forum.

Robert M. Wachter, MD, Professor of medicine at UCSF, associate chairman of UCSF's department of medicine, and chief of the medical service at UCSF Medical Center. He is a national leader in the field of medical errors and patient safety. He is editor of AHRQ WebM&M, a case-based patient safety journal on the Web, AHRQ Patient Safety Network, the leading federal patient safety portal, and "Quality Grand Rounds," a case-based series on medical errors in the Annals of Internal Medicine. Dr. Wachter was project director of Making Healthcare Safer: A Critical Analysis of Patient Safety Practices, produced for AHRQ and published in 2001. Nearly 100,000 copies of the report were distributed. He is the author of a recent book on medical errors, Internal Bleeding: The Truth Behind America's Terrifying Epidemic of Medical Mistakes. The book has received glowing reviews and has been a national bestseller. Dr. Wachter received one of the 2004 John M. Eisenberg Awards, the nation's top honor for contributions to the field of patient safety. In 2005, Modern Physician magazine named him one of the 30 most influential physician-executives in the United States.

Richard J. Liekweg, Chief Executive Officer of the University of California, San Diego (UCSD) Medical Center. UCSD Medical Center is a 540 bed academic medical center recently recognized as a top 100 Most Wired Hospital and a top 25 Most Wireless Hospital. Prior to joining UCSD Medical Center in 2003, Mr. Liekweg spent 16 years with the Duke University Health System. In 2001, Mr. Liekweg was featured as one of Modern Healthcare's “Up and Comers” in hospital administration. In 2003 he was named a Durham City of Medicine Ambassador which honors those whose presence enhances the city's prestige.

Dennis Jordan, Founding principal partner and Vice President of Suture Express, a specialty medical supply distribution business and currently the 3rd largest distributor of wound closure product in the US. He previously served as Director of Healthcare Services for Neoforma.com, the leading e-commerce solution company for the healthcare industry. Dennis is a licensed pharmacist and has served as Director of Materials Management at Miami Children's Hospital and Director of Pharmacy at Jane Phillips Memorial Medical Center – Bartlesville, Oklahoma.

Sherlyn Hailstone, President of SSM St. Joseph Health Center and SSM St. Joseph Medical Park since November, 2005. The facilities are members of the SSM Health Care System. St. Joseph Health Center is a 451 bed hospital over two campuses. St. Joseph in St. Charles with 365 acute care beds, St. Joseph–Wentzville in Wentzville is an 85 bed inpatient behavioral medicine facility with outpatient diagnostic and a 24/7 emergency department. St. Joseph Medical Park in St.Peters is a 100,000 square foot ambulatory center.Mrs. Hailstone previously served as the EVP/CEO of Saint Francis Hospital, a 400-bed acute care hospital located in Evanston, Illinois. Prior to joining Saint Francis in 2001, Mrs. Hailstone was the Chief Operating Officer for BroMenn Healthcare in Bloomington, Il. From 1975 until 1996 Sherlyn held various nursing and administrative positions at Barnes – Jewish Hospital in St. Louis, MO. She graduated from Barnes School of Nursing and earned her BSN and MSN in Nursing from St. Louis University.

Bill Williams, Special Venture Partner with Galen Partners. Mr. Williams was President of Pyxis Corporation from 1996-1998. He joined Pyxis in 1988 as the second employee and its Vice President of Marketing and Sales. Later he served as Vice President of Sales and Sales Operations, Senior Vice President and Executive Vice President. He played a prominent role in guiding Pyxis to one of the nation's most successful public offerings in 1992, and eventually a billion dollar merger with Cardinal Health, an S&P 500 company, in 1996.
